I-9 - Needed
  • Must be completed the Form I-9 no later than the first day of employment, but not before accepting the job offer.
  • If alien authorized to work ensure that the alien registration number and the work authorization expiration date are noted on the I-9
  • Ensure that the acceptable documents are correctly listed in section 2 and signed by the authorized official of the department. The date of signing should coincide with the date that authorized official reviewed original documents
  • If a non-resident alien is a visa holder, i.e. F-1 or J-1, attach a copy of the updated work authorization, a copy of the foreign passport and visa with the I-94 departure record. Any other additional Temporary Employment Authorization may need a petition. Please check handbook accordingly.

  • Sent electronically by the University

SprintTax Form - Needed
  • For F-1 Status: SprintTax Form is required in place of the W-4 and IT-2104

  • Employees identified as Authorized Aliens, i.e. H1/HIB Initial Appointment Letter provided to CPLA Department

Social Security Card - Needed
  • Required for Payroll Purposes Only

Amended Constitutional Oath - Needed
